A Justice Department report has unveiled systemic failures within the U.S. Bureau of Prisons, leading to the preventable deaths of nearly 200 inmates, with 187 of these being deaths by suicide over the course of 8 years, from 2014 to 2021. The report attributes these deaths to a combination of safety failures, including the widespread use of single cells, the inability to control the influx of drugs and weapons, operational failures, policy violations, deficiencies, missteps, negligence, and a blundering workforce. This crisis within the federal prison system is highlighted by the fact that half of the 344 deaths over this period were suicides. The findings come in the wake of the Justice Department carrying out 13 executions in the final six months of Donald Trump's presidency, sparking further scrutiny of the prison system.
