Cheshire Police said three former members of the senior leadership team at the Countess of Chester Hospital were arrested on 30 June on suspicion of gross negligence manslaughter. The unnamed suspects, who held executive roles at the hospital in 2015-2016, have been questioned and released on bail while the investigation continues.
The arrests form part of Operation Duet, a probe launched in October 2023 after nurse Lucy Letby’s convictions to examine potential corporate manslaughter at the hospital. In March 2025 the inquiry was widened to consider whether individual actions or inaction amounted to gross negligence manslaughter. Detectives are also reviewing deaths and non-fatal collapses at neonatal units in Chester and at Liverpool Women’s Hospital from 2012 to 2016.
Letby, 35, was found guilty in August 2023 of murdering seven infants and attempting to murder seven others during a 13-month period at the Countess of Chester Hospital’s neonatal unit. She is serving 15 whole-life sentences, and a separate public inquiry into the hospital’s handling of the deaths is ongoing.
